How do I transfer my medical records to Riverside Healthcare?

Prior to/on May 16, please contact Westwood directly and have medical records faxed to Riverside Women’s Health at (815) 935-2970.
After May 16, please contact St. Mary’s Hospital at (815) 937-2400 and have medical records faxed to Riverside Women’s Health at (815) 935-2970.
You may schedule your appointment before sending your medical records, however, records must be received before your visit.
