Behavioral Health Intake Process

Guiding you step-by-step through Riverside’s intake process for personalized mental health support and care.

Central Intake Department:

  1. Call (844) 442-2551 or go to Riverside Medical Center’s Emergency Room and ask for a mental health screening.
  2. If calling, the intake team will direct the individual to come to the Emergency Room or provide them with resources.
    1. If a referral is coming from an outside resource, a referral can be faxed to (815) 935-3329.
  3. Upon coming to Riverside’s Emergency Room, you will be greeted by the ER staff, including physicians and nurses, who will complete a medical evaluation.
  4. You will then be greeted by our Central Intake staff who will complete a mental health screening.
  5. When receiving the mental health screening, which will include the individual’s personal information and presenting problems.
  6. Based on this information, the intake team, which includes the evaluator, ER staff, and psychiatrist, will determine if the individual is in need of inpatient hospitalization or outpatient services. 
  7. Upon determining the level of appropriate care the intake team will provide the appropriate resources.

Inpatient:

  1. Behavioral Health’s inpatient units are located within Riverside Medical Center.
  2. The individual is transported to the inpatient unit for admission.
  3. Individuals will be greeted by their nurse and other mental health technicians who will attend to your medical and emotional well-being.
  4. The inpatient team will provide education on unit-specific programming.

Pathways (Outpatient Services):

  1. If appropriate for PHP (Partial Hospitalization Program) or IOP (Intensive Outpatient Program) services, a Pathway’s team member will reach out to schedule intake with the individual.
  2. Upon intake, the individual will come to Pathways at 400 S. Kennedy, Suite 200, Bradley, IL 60915.
  3. At the intake appointment, the individual will meet with a Pathway’s team member to complete the required consents, assessments, and treatment planning. They will be introduced to the programming on this day.
