Our Home Health Care program provides access to skilled professionals who work closely with your physician to develop a personalized care plan. Whether you need nursing services, therapy, social support, or daily living assistance, we’re here to help you recover and thrive right at home. Counties served include Kankakee and portions of Iroquois, Will, Grundy and Livingston county.

Our Home Health Care Team

Skilled Nursing

Skilled Nursing care is provided by a Registered Nurse (RN) or Licensed Practical Nurse (LPN) with specialized training in home-based care. Your nurse will:

• Communicate with your physician to update your plan of care.
• Perform skilled nursing procedures, such as wound care or medication management.
• Teach you, your family, and caregivers about your condition, self-care techniques, and prevention strategies.
• Coordinate care and services with your physician and other healthcare team members.

Physical, Occupational & Speech Therapy*

Our licensed therapists—or licensed therapy assistants under therapist’s direction—provide the following services to help you regain mobility, independence, and confidence at home:

• Customized exercises and treatment plans to rebuild strength, balance, and coordination.
• Speech and swallowing therapy to enhance communication and safety with eating and drinking.
• Clear instructions on how to continue therapy safely at home.

Coverage Note: Medically necessary therapy services must be coordinated with our agency while you are receiving Medicare-covered home health services. If you arrange these services independently while under our plan of care, you may be responsible for any related charges.

*Speech therapy currently unavailable until March 24.

Medical Social Services

Our Medical Social Worker is here to help you navigate life changes, manage stress, and connect with resources. Services may include:

• Short-term counseling for emotional or mental health needs.
• Coordination with community resources and support programs.
• Guidance on living arrangements, finances, and long-range planning.

Home Health Aide Services

When you need part-time help with personal care, our Home Health Aides can assist you under the supervision of a nurse or licensed therapist. Services are added to your plan of care based on your needs and may include:

• Bathing, shampooing hair, and dressing assistance.
• Changing bed linens and light household tasks related to personal care.
• Helping you with safe movement around the home and other daily activities.

Returning to everyday activities after an illness or injury can be a challenging process. Riverside Home Health Care can help make this transition safer and easier for patients and their families.

Remote Patient Monitoring & Telehealth Services

These services may be provided by audio or video to supplement in-person visits. Telehealth services must be related to your skilled care, included in your plan of care with specific goals to promote positive treatment outcomes, and ordered and approved by your physician. Through a partnership with Health Recovery Services (HRS), eligible patients have access to remote patient monitoring, which offers:

• Symptom surveys
• Medication reminders
• Health education
• Virtual visits
• Biometric monitoring and more!

Complete recovery often requires the services of many medical specialists. Riverside Home Health Care allows a seamless transition between hospital and home recovery by coordinating a team of home health professionals to carry out the physician-prescribed treatment plan. Nursing and therapeutic staff visit and care for patients in their homes and are available 24 hours a day for questions or problems.
